Give champion roles a purpose, a structure and something to show for them.
The Carevia Champion Strengths Profile helps managers review whether a champion is actually developing knowledge, influencing practice and supporting others, rather than simply holding a title.
It combines a core competency framework, role-specific strength statements, RAOF™ actions, progress tracking and a formal outcome decision in one browser-based review.
A champion badge does not tell you whether the role is working.
Champion roles can look useful on an organisational chart while making very little difference to day-to-day practice.
The important question is not whether someone has been named the medication, safeguarding or dementia champion. It is whether they understand the role, use that knowledge in practice, help colleagues develop and contribute to improvement.
The Champion Strengths Profile gives managers a structured way to review that.
- Review a consistent set of core champion competencies across every role.
- Add role-specific strengths relevant to the champion area being reviewed.
- Record evidence for Developing, Established and Leading ratings.
- Turn development needs into practical RAOF™ actions.
- Compare current and previous reviews to see where capability is progressing or slipping.
A structured champion review, not a generic supervision form.
The tool is designed specifically around how champion roles should contribute to workforce capability and service improvement.
Nine built-in champion roles
Review Dementia, Safeguarding, IPC, Medication, Dignity & Respect, MCA, Falls, Nutrition & Hydration, and Activities & Wellbeing champions.
Five core competency domains
Assess Knowledge & Understanding, Embedding Practice, Communication & Advocacy, Reflective Practice & Learning, and Leadership & Influence.
Role-specific strengths
Each champion area has its own strength statements so the review reflects what that role should actually contribute.
Three-level rating system
Use Developing, Established and Leading to distinguish between building capability, consistent practice and active influence on others.
Qualifications and training record
Record relevant qualifications, training status and development needs alongside the review.
RAOF™ Action Cycle
Development actions require the champion to Read evidence, Ask people, Observe practice and Feedforward what should change next.
Progress comparison
Reload a previous review and compare ratings side by side to identify strengths that improved, held steady or need attention.
Outcome decision
Record whether the person is confirmed as champion, confirmed with a development plan, or needs reassessment after development.
Print-ready review record
Generate a structured summary, print two signed copies and retain the JSON working file for the next review.
Want champion roles to do more than exist on paper?
Get Carevia Pro+ and use the Champion Strengths Profile to review capability, agree actions and track development over time.
One review from role expectations to development action.
Set up the profile
Record the champion, manager, service, review dates, relevant qualifications and any context for the review.
Select the champion role
The tool loads role-specific strengths alongside the common competency framework.
Rate core competencies
Review the strengths that should apply to every champion, including knowledge, communication, reflective practice and leadership.
Review role-specific strengths
Assess whether the champion is applying the knowledge and behaviours expected in their specialist area.
Build RAOF™ actions
For development needs, define what evidence should be read, who should be asked, what should be observed and what action should follow.
Record the outcome
Confirm the role, confirm it with an active development plan, or reassess once further development has taken place.
It tests whether a champion is influencing practice, not just attending training.
Training matters, but a certificate does not show whether the champion can translate knowledge into day-to-day practice or help colleagues improve.
The profile deliberately looks at both capability and influence, then turns gaps into actions that require evidence from the service rather than a simple task list.
One framework, nine different champion areas.
The same core expectations sit underneath each role, with additional strengths tailored to the specialist area.
Make champion reviews part of your evidence of staff development.
The Champion Strengths Profile is designed as a structured supervision and development record. It helps managers move beyond appointing champions and think about whether those roles are actually strengthening staff knowledge, competence and leadership across the service.
- Record evidence of competence and development needs.
- Link actions to observable practice rather than paperwork alone.
- Use previous reviews to show progression over time.
- Generate a signed record for the staff file and champion.
A practical champion review tool for care homes, supported living and domiciliary care.
If you are looking for a champion competency assessment for adult social care, a care home champion review template or a structured way to review medication, safeguarding, dementia, MCA, falls, IPC or dignity champions, this tool is designed for that purpose.
Rather than using a generic supervision template, the Carevia Champion Strengths Profile helps registered managers and quality teams review whether a champion has the knowledge, confidence and influence needed to contribute meaningfully to the service.
It can help organise evidence around questions such as:
- What should a champion role look like in adult social care?
- How do we assess whether a champion is competent?
- How do we evidence a champion's impact on staff practice?
- What development actions should follow a champion review?
- How can we track whether a champion is progressing over time?
This is not simply a champion checklist. It is a structured review and development tool for services that want champion roles to contribute to real workforce capability.
Your review data stays in your browser.
The tool operates entirely within the browser. Information entered into the Champion Strengths Profile is not transmitted to or stored by Carevia.
You can download a JSON working file, reload it at the next review and print a signed review record for your own organisational records.

Before you get started
Is the Champion Strengths Profile a standalone product?
The Champion Strengths Profile is included with Carevia Pro+. The call-to-action on this page takes you to the Pro+ checkout.
Which champion roles are included?
The tool currently includes Dementia, Safeguarding, IPC, Medication, Dignity & Respect, MCA, Falls, Nutrition & Hydration, and Activities & Wellbeing.
How are champions rated?
Each strength can be rated Developing, Established or Leading. The review covers both common champion competencies and strengths specific to the selected champion role.
What is a RAOF™ action?
RAOF™ stands for Read, Ask, Observe and Feedforward. Instead of setting a simple task, the action asks the champion to gather evidence from written records, conversations and observed practice, then decide what should happen next.
Can I compare one review with another?
Yes. A previously saved Carevia JSON file can be loaded so current ratings and previous ratings can be compared, and previous actions can be reviewed for progress.
Can the review be printed?
Yes. The tool generates a print-ready review record with space for manager and champion signatures. The working JSON file can also be saved for use at the next review.
Where is the data stored?
The tool runs in your browser. Data is not transmitted to or stored by Carevia. You are responsible for securely handling saved and exported files.
