What will it reveal?

Three questions that completed audits do not always answer

The check is designed to make experienced managers and provider leaders pause. It tests the quality of assurance behind the paperwork rather than rewarding the number of audits completed.

1

Are your findings credible?

Explore whether audits explain why a standard is met and whether the evidence would withstand constructive challenge.

2

Does practice match the promise?

Consider whether policy, records, staff explanations, people’s feedback and observed practice tell the same story.

3

Did improvement actually work?

Test whether actions were merely closed or whether someone returned to the evidence and confirmed that the change became embedded.

A practical introduction to RAOF™

Follow one requirement through the evidence

RAOF™ is not four unrelated audit activities. It is one triangulation thread. You begin with what should happen, test how the same requirement is understood and experienced, then compare it with what happens in ordinary practice.

Where those sources do not align, Feedforward turns the contradiction into an action and a clear plan to test the same thread again.

What you receive

Useful actions, not a vanity score

This is not a compliance rating.

Your results show where confidence appears supported, where stronger testing is needed and where a clear response may be required. The purpose is to help you decide what to do next.

  • Three tailored priorities
Focused on the answers most likely to affect assurance and provider oversight.
  • A rationale for each answer
Capture why you chose it rather than reducing professional judgement to a tick.
  • Recommended practical actions
See what to test, clarify, improve or escalate next.
  • A downloadable action record
Keep a dated copy for reflection, discussion or governance follow-up.
  • A manager-to-NI sharing route
Open a ready-written email explaining the priorities, provider relevance and practical difference.

Service confidence and provider confidence are not the same question

The Reality Check adapts its questions so managers can examine service-level auditing while nominated individuals and provider leaders test the quality of oversight across services.

For registered managers

Test whether your audits establish what is really happening, identify the gaps that deserve attention and create a clear explanation you can take to your nominated individual.

For nominated individuals and provider leaders

Explore whether service-reported assurance can be compared, independently challenged and followed through until there is evidence that improvement worked.

Questions about the free check

Is the tool genuinely free?

Yes. There is no payment and no card information is requested. Carevia Pro+ and Provider Licences are explained where they can support an identified gap, but purchasing is not required to complete the check.

Does it give my service a rating?

No. Ten reflective answers cannot produce a credible service rating. The tool identifies assurance signals, areas that need stronger testing and three practical priorities.

What happens to my answers?

Your assessment answers remain in your browser while you use the tool. Carevia receives the information entered into this sign-up form, but not your assessment responses or rationale.

Which services can use it?

It is designed for adult social care leaders in residential care, nursing care, supported living and homecare. The questions focus on governance and assurance rather than one service model.

What happens after I sign up?

The tool opens immediately after you submit the form. Confirm your email address using the message sent to your inbox and Carevia will also send the access link for future use. Check your spam or promotions folder if it does not arrive.

Can I share it with my nominated individual?

Yes. Managers can open a ready-written email from their results explaining the priority questions, why they matter at provider level and how the recipient can complete the check themselves.

Start with an honest question
